3. Once you have received your child’s PPSN and GMS number please phone or email us at skreenfamilypractice@gmail.com with these details so we can add them to their file.

 

​4. Book vaccination appointments with Nurse Niamh by telephone 071 9166619

please bring Calpol and Immunisation Booklet along to these appointments. (You will receive the immunisation booklet and orange health record book from your Public Health Nurse.)

5. Your local Public Health Nurse will contact you to arrange developmental check-up appointments.

6. Parent and Toddler group (0-36 months), every Thursday 10.30-11.30 am Skreen Dromard Community Centre 3euro per family
