New Patient Information Skreen Family Practice January 2026​
We are welcoming patients in our catchment area to our practice
To join our practice please follow these steps
1. Ring reception 071 -9166619 and give your basic details

2. An appointment with our nurse will be organised – the purpose of this is to introduce you to Skreen Family Practice. No clinical questions will be addressed during this consult.
​
3. Get your medical notes from your previous G.P.
​
4. Organise the transfer of medical card (if applicable)
When all the above has occurred an appointment with the doctor can be organised

Anyone age 8 to 69 can apply for a GP visit card - It's possible that you may qualify for a GP visit card even if the amount of money you earn is high, if your expenses are also high, you could still qualify.If you have a child under 8 please register for an U8s GP visit cardIf you are over 70, you can register for an over 70s GP visit card​
