Has your child had an anaphylactic reaction to a previous dose of influenza (flu) vaccine or any other vaccine or egg or any medications.
Has your child had an attack of asthma (increased wheeze or needed additional use of his/her inhaler in the last 72 hours? Has severe asthma: is your child on steroid tablets or has he/she had to be in ICU/ITU in hospital in the past?
Has your child significant immunosuppression (weakened immune system) due to disease or treatment
Does your child use aspirin/salicylates
Does your child live with a person who is immunocompromised ( a person who has a severely weakened immune system)
Has your child has significant immunosuppression (weakened immune system) OR has neutropaenia ( a low white cell count on blood test)
Do you consent for your child to have the nasal flu vaccine

Dear Parent/Guardian,

Dr Dunne and Nurse Kilcullen from Skreen Family Practice are undertaking a childhood flu vaccine clinic in the school on Friday 25th October.

If you would like your child to receive the nasal flu vaccine (this is a nasal spray, no needles involved) please fill in the following form.

A few points to note

1.The nasal flu vaccine contains a very small amount of ovalbumin (egg protein). The vaccine can be given to children with confirmed egg anaphylaxis or egg allergy in a primary care setting but children who have required critical care admission to hospital for a previous severe anaphylaxis to egg should be given the vaccine in a hospital setting.

2.We need your child’s PPS number in order to keep the national records up-to-date and avoid duplication. All information given will be kept in strictest confidence and in compliance with GDPR.
